JOB PURPOSE:

To work with Neighbourhood Services to identify residents experiencing mental health challenges that are impacting their tenancy and to carry out the statutory duties under the Care Act and the Mental Health Act, where qualified to do so to support the resident, reduce the impact on the community where anti-social behaviour is evident and minimise the risk of legal action; and to work within the Mental Health Locality teams to improve outcomes for vulnerable residents with mental health problems.

DESCRIPTION OF DUTIES:

• To work within the existing Mental Health Locality teams as a team member and from their team bases North and South, performing duty roles under the oversight of the Locality Team managers.
• To work with Neighbourhood Management departments to identify residents/households who require support with their mental health.
• To support residents with their mental health to sustain their tenancies through establishing a support framework in conjunction with partner agencies.
• To establish and maintain effective relationships with clients, service providers and partner agencies to facilitate referrals and to meet identified needs.
• To assist the Neighbourhood Teams in managing vulnerable tenants with mental health needs which may include supporting perpetrators, victims and witnesses of anti-social behaviour.
• To conduct mental capacity assessments under the Mental Capacity Act for residents who demonstrate concerns about their capacity to engage with their tenancy, make decisions or engage with the court process.
• To undertake assessments of need and risk assessments under the Care Act where required, working with service users to establish eligibility, generate care and support plans, including crisis care plans and personal budgets, and to take part in reviewing these.
• To work with residents who are victims of self-neglect and hoarding. To assess hoarding scale and, if necessary, manage through the safeguarding processes. To conduct necessary assessments and support them through the relevant panel to promote their care and support.
• To assess individuals housing and support needs and draw up individual support plans in conjunction with partner agencies where appropriate to address housing related problems and review at regular intervals.
• To identify Adult Safeguarding and Child Safeguarding issues and to implement procedures as required. This may include key duties under both of these procedures, i.e., investigating and managing if appropriate, as Lead Enquiry Officer and Safeguarding Manager if so trained.
• To maintain relevant up to date records of all work undertaken in accordance with Trust and Local Authority policy and procedure, including data entry on all relevant IT systems.
• To attend and use supervision sessions with line manager and team professional supervisor effectively, including in relation to workload management and personal development.
• To keep abreast of legislation, and both government and departmental policies and procedures.
• To attend local social work forums; this may include AMHP forums or departmental specific forums related to the social work role.
• To attend and contribute effectively to multi-disciplinary team meetings, liaison groups, case reviews and other appropriate meetings.
• To operate within the corporate lone working policy

Qualifications
– Holder of a social work qualification (CQSW, Dip.SW or equivalent) and AMHP accreditation
– Approved Mental Health Professional
Skills; Experience and Attitude
– Able to meet the registration criteria set down by the HCPC.
– Experience of working with adult service users with Mental Health problems, from a range of ethnic and cultural groups.
– Post qualified experience of working with people with severe and enduring mental health problems.
– Experience of undertaking mental capacity assessments.
– Experience of working in multi-disciplinary team.
– Knowledge and understanding of the various levels of disadvantage which may impact in diverse communities.
– Knowledge and understanding of current legislation and guidance, including Mental Health Act 1983 and the Care Programme Approach, Care Act 2014, Mental Capacity Act 2005, Children Act 1989.
– Ability to communicate clearly and effectively, in writing and verbally, with service users, carers and other professionals.
– Ability to apply the principles of equal opportunities to the duties of the post. Knowledge and understanding of housing and community safety legislation applying in the role of supporting individuals and communities under stress in these areas.
