The post of a social worker is to support the discharges of residents of the borough from the local acute, out of borough hospitals and the local Rehabilitation Unit. The team works in line with the Discharge to Assess model of care to arrange services to assist with the recommended discharge pathways. The applicant should have previous experience in adult social services, with a keen interest in hospital discharges. Have strong skills in multidisciplinary work, assessment, undertaking Mental Capacity Assessment, including excellent knowledge of the relevant social care legislation, safeguarding and of the continuing healthcare process.
