Adult Social Care Commissioning Manager – Project

Duration – up to 6 months (subject to review)
Context
● The majority of Kingston’s reablement services is provided by Your Healthcare
Community Interest Company (YHC) through a block contract.
● If additional reablement is required, this is delivered through multiple home care
providers, via the Homecare Framework Lot 2 (Reablement).
● We have completed a Reablement Review, which has found a number of areas of
service improvement as well as recommendations to change the wider enablement
model. This now needs urgent progression and delivery.
● Developing reablement services is a key transformation priority as part of the
‘Promoting Independence’ workstream.
Purpose
The role will develop and deliver solutions to ensure that Kingston is demonstrably
maximising the effectiveness, impact and quality of its reablement offer, to prevent,
reduce and delay the need for long term care.
The key outputs for the role over the 5 month period are:
1. Develop a proposed new service model to implement over medium term
2. Develop an update set of detailed recommendations and implementation plan to
deliver improvements to the Reablement services in 2025/26 (addressing some of
the issues identified in the reablement review).
This will require significant levels of engagement with people who access services, front-line
works, social workers, health partners and commissioners.
The reablement review from 2023 will act as the baseline to start the next phase of the work,
which needs to develop practical and implementable solutions.
Key tasks and responsibilities
● To review the Reablement Review findings and recommendations and current draft
improvement plan, identify key issues and areas for improvement as well as lines of
enquiry for further research. This will need to cover:
○ User Journeys / Pathways
○ Data & outcomes measures
○ Quality & experience
● To seek additional intelligence & insight (both qualitative and quantitative) on
performance (as well as areas for improvement & developments) since late 2023 (the
date the reablement review was completed)
● To undertake further research on lines of enquiry / gaps in original research to
improve understanding and support recommended improvements / future service
model.
● To undertake research with people who access services, front-line workers, social
workers, health practitioners and commissioners to develop a new service model and
improvement plan
● To review good practice locally and nationally – with focus on models with high quality
items (ideally detailed discussions and analysis of existing high performing services)
● To conduct a review of the current delivery model (inc. the commissioning model) and
develop options for future service model (including considering value for money,
outcomes & impact, good practice, risks & strategic drivers) as well as an
implementation plan for improvements.
Umbrella pay rate – £430.37
