Role Overview
This is a short-term, project-based role designed to support hospital discharge pathways for patients who are homeless or at risk of homelessness. The postholder will work across hospital and council sites to deliver housing advice, discharge coordination, and statutory homelessness assessments.
Key Responsibilities
- Attend hospital multi-disciplinary team (MDT) meetings and coordinate housing-related discharge planning.
- Provide housing options advice and support to patients with complex needs.
- Conduct homelessness assessments and produce statutory decision letters under Part 7 of the Housing Act 1996.
- Write and issue Section 184 notifications in line with legal requirements.
- Liaise with NHS, Adult Social Care, and housing teams to secure appropriate accommodation.
- Ensure timely referrals and case progression in line with the Homelessness Reduction Act 2017.
Essential Requirements
- Experience working with homeless clients, ideally in a hospital discharge or housing advice setting. Strong working knowledge of homelessness legislation, including the Housing Act 1996 and Homelessness Reduction Act 2017.
- Proven ability to write statutory decision letters, including Section 184 notifications.
- Ability to manage complex cases and work under pressure in a fast-paced, multi-agency environment.
- Excellent communication and partnership working skills.
- Comfortable working across multiple sites, including hospitals and council offices.
- Additional Information This role is office-based with regular presence required at hospital sites.
- Candidates must be available to start immediately and able to commit to the full 6-month assignment. DBS clearance required.
| JOB SUMMARY: | To be responsible for the provision of high-quality housing options, advice, and assistance to patients in the hospital.
Must have a good working knowledge of Homelessness Legislations and the provisions. The Homelessness Reduction Act 2018
To work with the hospital team to facilitate safe and timely hospital discharge into suitable and appropriate accommodation for all Tower Hamlets residents. To work with the Hospitals/routes to routes team to refer clients to their borough of residence.
To support discharges from hospitals for all Tower Hamlets residents who need social care or housing support from: • Royal London Hospital • Newham • Mile End • Any other hospital that refers TH residents to the borough To be experienced in homelessness legislation and have a detailed understanding of the housing sector and an understanding of the importance of housing in reducing demand on hospitals and social care.

To provide homelessness prevention, housing options and bespoke service to those single clients homeless or threatened with homelessness
To assist and guide single clients with support needs to make appropriate choices within their housing pathways
To ensure that all enquiries and cases are progressed speedily and efficiently and that all set targets are achieved |

| 1. | To act as the first and initial point of enquiries and information for all housing related discharge referrals where it is assessed that the patient will be homeless on discharge, or the accommodation will not be suitable.
|
| 2. | To work with the CCG to ensure and effective and efficient discharge processes to prevent re-admissions, and support and inform wider system-change and commissioning activity.
|
| 3. | To work in partnership with a wide range of agencies, including but not limited to: Adult Social Care, Through Care
Services, Rough Sleeping team, Supported Accommodation, GPs, occupational therapists, care providers, Probation Neighbourhood Teams, housing and benefits teams to determine specific housing needs and the most suitable housing options for clients |
| 4. | To use knowledge of Housing Related Support (HRS) to provide a high-quality housing options, advice and assistance to patients on the ward that have HRS needs

| 5. | To use a dedicated assessment tool to determine the most appropriate form of supported accommodation for single clients
|
| 6. | Ensuring the efficient functioning of the Tower Hamlets Hostel and Temporary Accommodation Pathway about rough sleepers including coordinating and facilitating all moves in and through the Pathway in line with strategic priorities. |
