London borough of Havering are looking for an Tenancy support officer To offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following discharge from hospital.
• To provide a high quality service to clients who are housed in accommodation who have complex needs individuals.
• To support people to sustain their quality of life, develop support plans and engage various support agencies. Provide advice for safeguarding and health and wellbeing on a wide range of issues including housing, welfare rights, personal finance, hoarding, tenancy management, mediation, life skills and the promotion of personal independence.
• Act as the principle point of contact for casework within their accommodation and to co-ordinate solutions for drug and alcohol, domestic violence, and mental health issues.
• To ensure environment and condition that clients experience is high quality and well maintained.
• To support on safeguarding and vulnerabilities to ensure smooth transition into suitable accommodation.
• Where relevant take action in line with license agreements, tenancy or leases, manage nuisance and anti-social behaviour casework.
• To develop strong links with other Council departments to provide solutions such safeguarding and vulnerability to enable the clients to maintain properties in good order and protect residents.
• Involvement in assessments to help co-design a person centred, strength-based support plan according to their needs;
• To Provide intensive one-to-one support and consistent contact to clients
• Adopt a multi-disciplinary approach with a focus on coordinating support. Make connections with a wide range of support services (including specialist services
such as mental health support and substance misuse treatment) to facilitate support and assistance for individuals to address social and health issues and other personal barriers to sustaining housing;
• Facilitate joint working and develop initiatives with other Voluntary Agencies (e.g. day centres), Local Authority Agents (e.g. Parks workers, Wardens), Health Providers (GP surgeries, A & E ), Prisons and Discharge services and the Police (e.g. SNT, PCSOs) as appropriate to meet service requirements, client aspirations and community need.
• Identify and report back to the Specialist Team, any changes in services in the wider system which are needed to improve support given to the target cohort of customers;
• Record output and outcome monitoring in line with Team procedures
