Job Details
Work Pattern: 37 hours per week,
PAYE Pay Rate: £16.62 per hour
Main Purpose
To support young people aged 16+ who are leaving care, ensuring the Council meets its responsibilities under the Children (Leaving Care) Act 2000. The role focuses on promoting independence, supporting care leavers and helping them access appropriate services and opportunities.
Typical Day
- Manage a caseload of care leavers and support their transition from care.
- Develop, review and implement Pathway Plans.
- Provide practical support to help young people achieve independence.
- Advocate for young people and ensure they receive their appropriate entitlements and services.
- Coordinate support across housing, education, employment, training and health services.
- Maintain regular contact with care leavers and monitor their progress.
- Identify and respond appropriately to safeguarding concerns.
- Maintain accurate electronic case records and documentation.
- Encourage young people to participate in their own planning and support.

Essential Experience & Knowledge
- Experience working with vulnerable young people, including those experiencing difficult or crisis situations.
- Experience developing positive working relationships with young people.
- Experience involving young people in problem solving and planning.
- Knowledge of statutory processes relating to Cared for Children and Care Leavers.
- Knowledge of Children Leaving Care legislation and Local Authority responsibilities.
- Understanding of independence preparation and training.
- Ability to assess needs and independence readiness and develop appropriate plans.
- Ability to communicate effectively with young people, colleagues and partner agencies.
- Ability to priorities workload and meet tight deadlines.
- Commitment to safeguarding, equality and diversity.
- Willingness to work in line with the Rochdale Way values.
- Travel: Required within and outside the borough; casual user allowance available for car owners
- DBS: Enhanced DBS and background checks required
