Children’s Social Worker – Children with Disabilities (CWD)
Location: Herefordshire
Contract: Permanent, Full-time (37 hours per week)
About the Role
We are looking for a passionate and dedicated Social Worker to join our Children with Disabilities Team, supporting children and young people with a range of disabilities and additional needs to achieve the best possible outcomes.
As a Children with Disabilities Social Worker, you will work directly with children, young people and their families, carrying out holistic assessments, developing care and support plans, coordinating multi-agency services, and ensuring children receive the right support at the right time. You will build strong relationships with families, promote independence, and ensure children’s voices remain at the centre of all decision-making.
Working within a supportive specialist team, you will collaborate closely with professionals across health, education and social care while maintaining a strong focus on safeguarding, wellbeing and positive outcomes.
Key Responsibilities
- Manage a caseload of children and young people with disabilities.
- Complete statutory assessments and develop child-centred care and support plans.
- Work collaboratively with children, families and partner agencies to coordinate effective services.
- Safeguard children by identifying and responding appropriately to risks and concerns.
- Build positive, trusting relationships with families using a restorative and strengths-based approach.
- Attend and contribute to multi-agency meetings, reviews and planning processes.
- Monitor, review and adapt support plans to ensure children’s changing needs are met.
- Maintain accurate and timely case records in line with statutory requirements.
About You
To be successful in this role, you will have:
- A recognised Social Work qualification (Degree, DipSW or CQSW).
- Current registration with Social Work England.
- Experience working with children, young people and families within statutory children’s services.
- Knowledge of safeguarding legislation, assessment frameworks and child-centred practice.
- Experience completing assessments and coordinating multi-agency support.
- Excellent communication, relationship-building and organisational skills.
- The ability to manage complex situations with resilience, professionalism and sound judgement.
- A commitment to delivering inclusive, empowering and person-centred support for children with disabilities and their families.
