Role Purpose

The Move-On Coordinator (Hospital Discharge) is part of the Mulberry Junction team and is responsible for supporting individuals affected by homelessness to achieve positive, sustainable move-on into longer-term accommodation.

The post holder will coordinate rapid and effective discharge from hospital settings, ensuring that individuals experiencing homelessness are supported to transition safely into appropriate accommodation. The role will contribute to improved health, wellbeing, and housing outcomes while preventing rough sleeping and delayed hospital discharge.

Working in partnership with health professionals, housing teams, social care, and voluntary sector partners, the post holder will actively participate in discharge planning, assessments, safety planning, and ongoing resettlement support.

Key Responsibilities

  1. Work with hospital in-patients experiencing homelessness who have a link to the borough, including those with determined or undetermined housing status and those with No Recourse to Public Funds (NRPF), to prevent rough sleeping and delayed discharge.
  2. Take an active role in hospital discharge planning by undertaking assessments, coordinating support plans, and facilitating smooth transitions into community accommodation.
  3. Facilitate multi-agency case management meetings to ensure coordinated support, enabling clients to access health services, wellbeing support, employment or volunteering opportunities, community organisations, and immigration advice where required.
  4. Support housing needs colleagues by contributing to Duty to Refer (DTR) investigations, gathering relevant information, and exploring appropriate housing options including step-down accommodation.
  5. Provide time-limited resettlement support following discharge to help individuals sustain their new accommodation.
  6. Conduct comprehensive Move-On Assessments to identify goals, housing options, health needs, and wider support requirements.
  7. Work closely with Local Lettings and Temporary Accommodation Teams to secure private rented sector or other suitable housing solutions.
  8. Liaise with supported housing providers, hospital teams, outreach services, and key stakeholders to enable improved move-in processes and successful transitions.
  9. Attend and contribute to weekly housing and hospital discharge planning meetings, Multi-Disciplinary Team (MDT) meetings, complex case discussions, and reflective practice sessions.
  10. Work collaboratively with Adult Social Care, including the dedicated Rough Sleeping Social Worker, to ensure eligible individuals access entitlements under the Care Act 2014.
  11. Maintain accurate case records and contribute to financial monitoring and reporting requirements.
  12. Collect, analyse, and provide relevant performance data and outcome information to support service improvement and system change.
  13. Undertake outreach and peripatetic working, including hospital in-reach and visits to community accommodation settings.

Knowledge, Skills and Experience

Essential

  • Good knowledge of the Homelessness Reduction Act 2018 and the Care Act 2014.
  • Knowledge of the UK welfare benefits system and rights of EEA nationals.
  • Knowledge of the EU Settlement Scheme process.
  • Experience of presenting data and information clearly and appropriately.
  • Commitment to treating people affected by homelessness with respect and dignity.

Desirable

  • Experience supporting individuals to move from temporary or supported accommodation into private rented housing.
  • Experience working with single homeless adults and/or people affected by rough sleeping.
  • Experience reviewing case management and key-working practice to ensure quality outcomes.
  • Experience using CHAIN or other homelessness case management systems.
  • Ability to use initiative and apply creative, solution-focused approaches to complex situations.

Working Environment

  • Primarily office-based with regular hospital in-reach and community visits.
  • Regular use of computer systems.
  • Exposure to hospital environments, including potential exposure to infectious diseases.
  • No line management responsibility.
