Job Title: Occupational Therapist
Team: HASS Cranbrook & Loxford (Adult Social Care)
Location: Locality-based (community role)
About the Role
We are seeking an experienced Occupational Therapist to join a locality-based, multi-disciplinary team delivering integrated community health and adult social care services.
You will manage a caseload of approximately 4 cases per week, working with individuals with a range of health conditions to promote independence, safety, and wellbeing within their own homes.
Key Responsibilities
- Undertake holistic, person-centred assessments and reviews
- Assess for and recommend simple and complex equipment
- Design and specify major housing adaptations, including preparing sketches and interpreting plans
- Provide guidance on manual handling and safe working practices
- Develop and review support plans in collaboration with service users and carers
- Work effectively within a multi-disciplinary team
- Manage and prioritise a complex caseload independently
Essential Requirements
- Professional Occupational Therapy qualification and registration with the Health and Care Professions Council
- Strong experience with major adaptations, specialist equipment, and manual handling
- Experience working in adult social care or community settings
- Ability to undertake complex assessments and manage cases autonomously
- Good understanding of social care legislation and housing frameworks
- Experience working within a multi-disciplinary team
