Occupational Therapist – Community Services (Porth Ceredigion)

About the Role
We are looking for a skilled and experienced Occupational Therapist to join the Porth Ceredigion community team.
You will work with adults and older people in the community, helping them to maintain or regain independence, safety, and quality of life in their own homes.
You will be part of an integrated service delivering person-centred, evidence-based occupational therapy, working closely with health, social care, voluntary, and housing partners.
Key Responsibilities
- Manage a complex community OT caseload independently
- Carry out functional assessments in people’s homes
- Recommend and arrange:
- Specialist equipment
- Housing adaptations
- Carry out manual handling risk assessments and care planning
- Support service users and carers to achieve goals around:
- Self-care
- Daily activities
- Independence and wellbeing
- Provide expert advice on disability, environments, and assistive equipment
- Work collaboratively with multidisciplinary and external agencies
- Supervise and delegate tasks to OT assistants and support staff
- Support students and contribute to practice education
- Participate in duty rotas when required
Practice & Professional Standards
- Maintain HCPC registration and comply with professional standards
- Follow RCOT professional guidelines
- Adhere to safeguarding policies and relevant legislation including:
- Mental Capacity Act
- Social Services and Wellbeing (Wales) Act
- Safeguarding procedures
- Use evidence-based practice and continuous professional development
- Maintain accurate records and reports
Additional Requirements / Working Conditions
- Regular lone working in the community
- Home visits across varying environments and conditions
- Exposure to challenging or distressing situations
- Manual handling and use of specialist equipment
- Ability to travel independently (driving required)
- Flexible working to meet service needs
Essential Requirements
- Degree/Diploma in Occupational Therapy
- HCPC registration
- Experience in community OT with adults/older people
- Experience managing complex caseloads
- Strong knowledge of equipment and housing adaptations
- Ability to work independently and make clinical decisions
Desirable
- Experience in social care/community integration settings
- Experience supervising staff or students
- Fieldwork educator training
- Membership of professional body (RCOT/BAOT)
