About the Role

We are seeking a skilled and motivated Occupational Therapist to join a Locality-Based Multi-Disciplinary Team, supporting the Discharge to Assess (D2A) pathway across the Redbridge borough.

This is an exciting opportunity to work in a dynamic community setting where you will support individuals recently discharged from hospital or at risk of admission, helping them regain independence and remain safely in their own homes.

You will manage a varied caseload of approximately 4 new cases per week, including both standard and complex Occupational Therapy assessments.


Key Responsibilities

  • Undertake holistic, person-centred OT assessments for adults with diverse and complex needs
  • Develop, implement, and review tailored support and care plans
  • Assess for, prescribe, and review specialist equipment and home adaptations, including complex seating and manual handling solutions
  • Provide training and guidance to service users, families, and carers on safe use of equipment
  • Work within legal and social care frameworks, including relevant housing legislation
  • Manage and prioritise a complex caseload independently
  • Collaborate effectively within a multi-disciplinary team (MDT) including health and social care professionals
  • Support safe and timely hospital discharge pathways

Essential Requirements

  • HCPC registration as an Occupational Therapist
  • Recognised degree/diploma in Occupational Therapy
  • Strong experience in specialist equipment, seating, manual handling, and adaptations
  • Proven ability to complete complex OT assessments and care planning
  • Sound understanding of social care legislation and housing adaptation processes
  • Experience working within a multi-disciplinary team
  • Excellent caseload management, prioritisation, and autonomous working skills

Desirable Skills

  • Experience within community OT or hospital discharge services
  • Confidence managing complex cases independently
  • Strong communication and report-writing skills
  • Experience delivering equipment training to carers and service users
