PAYE Pay Rate: £36,363.00 yearly

Main Purpose
To support children and young people with disabilities and their families by assessing their needs, coordinating support and ensuring their safety, wellbeing, independence and voice are at the centre of decisions.
A Typical Day
- Assess children and families’ needs.
- Develop and review support plans.
- Work with families and carers.
- Coordinate with health, education and social care.
- Manage complex cases.
- Make child-centred decisions.
- Promote independence and positive outcomes.
- Attend supervision and reflect on practice.
- Seek guidance when dealing with complex situations.

Essential Qualifications
- Registered with Social Work England.
- Social Work Degree, DipSW or CQSW.
