Supporting Care Leavers aged 16-25 as they transition into adulthood.
Responsibilities include:
-Direct work and intervention supporting preparation for independence
-Asessment, planning and review (Pathway Plans)
-Working with partner agencies in DWP, Housing, Health and Education to achieve the best outcomes for care leavers
-Deliver group work
Umbrella hourly rate – £24
