- Follow individual care and support plans, monitor progress and contribute to assessments and reviews.
- Assist residents with mobility, transfers and prescribed exercise programmes, following appropriate risk assessments and moving-and-handling procedures.
- Prompt and support residents with prescribed medication in accordance with their care plans and council policies.
- Monitor changes in residents’ health, mood, behaviour or circumstances and report concerns to relevant health and social care professionals.
- Liaise with residents, families, carers and multidisciplinary teams to ensure coordinated support.
- Maintain accurate daily records and case notes, ensuring confidentiality and compliance with data protection requirements.
- Follow safeguarding, health and safety, lone-working and Care Quality Commission (CQC) requirements.
- Undertake relevant training and participate in supervision, team meetings and multidisciplinary reviews.
Essential Skills and Experience
- Experience supporting adults with personal care, daily living activities or social care needs.
- Ability to promote independence and work towards personalised reablement goals.
- Good communication and interpersonal skills, with a respectful and person-centred approach.
- Ability to recognise changes in residents’ wellbeing and escalate concerns appropriately.
- Understanding of safeguarding, confidentiality, health and safety, and moving-and-handling procedures.
- Ability to manage workload, work independently and as part of a multidisciplinary team.
- Flexibility to work across the borough and respond to changing service needs.
Qualifications and Training
- Level 3 Diploma in Health and Social Care, or willingness to complete the required qualification.
- National Care Certificate, or willingness to obtain it.
- Willingness to complete mandatory training and further development, including relevant reablement and care-related training.
Ideal Candidate
The successful candidate will be compassionate, patient and reliable, with a genuine commitment to helping adults regain their confidence and independence. They will be comfortable providing hands-on personal care, working in residents’ homes and collaborating with health and social care professionals to deliver safe, effective and person-centred support.
