Social Worker – Adults, Health Partnerships and Housing
Location: Kettering / Wellingborough
Rate: £33–£35 per hour, depending on experience
Hours: 37 hours per week
Contract: 6–9 months
Working Pattern: Initially office-based for induction, then hybrid working once established in the role
About the Role
We are seeking an experienced Adult Social Worker to join an integrated Health, Social Care and Housing service. You will manage an allocated caseload of adults with care and support needs, completing assessments, managing risk and arranging appropriate support packages.
You will work closely with customers, carers, families, health professionals, housing services, hospitals, brokers and third-sector organisations to deliver personalised, outcomes-focused support.
Key Responsibilities
- Complete Care Act assessments, reviews and risk assessments for adults and carers.
- Manage an allocated caseload in line with statutory requirements, council policies and professional standards.
- Develop and coordinate appropriate, proportionate and cost-effective care and support plans.
- Work with brokers to arrange care packages, equipment and adaptations.
- Safeguard vulnerable adults and respond appropriately to identified risks.
- Apply relevant legislation including the Care Act 2014, Mental Capacity Act and Mental Health Act.
- Work effectively within a multidisciplinary and integrated Health, Social Care and Housing environment.
- Maintain accurate and timely case records, reports and documentation.
- Liaise with hospitals, specialist services, health professionals, housing teams and voluntary-sector organisations.
- Support and supervise less experienced colleagues and Assessment & Enablement Workers where required.
- Attend and lead multidisciplinary meetings where appropriate.
Essential Requirements
- Social Work degree, CQSW, DipSW or equivalent.
- Current Social Work England registration.
- Strong experience working within Adult Social Care.
- Experience completing Care Act assessments and managing adult social care caseloads.
- Knowledge of safeguarding, risk assessment and relevant legislation.
- Experience working with adults with needs such as mental health, older people, learning disabilities or physical disabilities.
- Strong communication, assessment and professional decision-making skills.
- Ability to work independently and as part of a multidisciplinary team.
- Full driving licence and ability to travel independently.
- AMHP training or experience would be desirable.
