Job Details
We are seeking an experienced Adult Social Worker to join the Transition Team, supporting young people aged 18+ as they move into and progress within adult social care services.
This role focuses on individuals who have an Education, Health and Care Plan (EHCP) and require ongoing care and support under the Care Act 2014.
Key Responsibilities:
- Undertake Care Act assessments for young adults (18+)
- Develop and review person-centred support plans
- Complete Mental Capacity Assessments and Best Interest decisions in line with legislation
- Work collaboratively within a multi-disciplinary team, including education, health, and partner agencies
- Participate in safeguarding processes and undertake court work where required
Requirements:
- Qualified Social Worker with experience in Adult Social Care
- Strong knowledge of the Care Act, Mental Capacity Act, and post-18 transition pathways
- Experience completing assessments, care planning, and risk management
- Ability to work confidently with multiple agencies and stakeholders
Working Pattern:
- Monday to Friday, 9:00am – 5:00pm
- Hybrid working available
DBS Requirement:
- Enhanced DBS – including barred lists for both children and adults
