Job Title: Mental Health Social Worker (Adult Social Care – Community Team)

Location: Camden (5 Pancras Square + community-based work)

Job Purpose

To provide high-quality, strengths-based social work support to adults with mental health needs, working within an integrated multidisciplinary team. You will promote recovery, independence, and safeguarding while delivering statutory social care under the Care Act and mental health legislation.

Key Responsibilities

  • Manage a personal caseload of adults with mental health conditions in the community
  • Complete Care Act assessments, including mental capacity assessments, care and support plans, and reviews
  • Undertake safeguarding duties, identifying and responding to risk, abuse, or neglect
  • Work collaboratively in multidisciplinary teams with health, housing, and voluntary sector partners
  • Act as key worker, coordinating care and ensuring timely support and interventions
  • Support carers through assessments, conversations, and care planning
  • Contribute to Court of Protection work where required
  • Participate in Mental Health Act processes and AMHP duty rota (training provided if needed)
  • Maintain accurate case records in line with statutory and organisational standards
  • Engage in supervision, reflective practice, and continuous professional development

Requirements

  • Qualified Social Worker registered with Social Work England
  • Knowledge of the Care Act 2014 and mental health legislation
  • Ability to assess risk, make sound professional judgements, and manage a caseload
  • Commitment to safeguarding and person-centred practice
  • Willingness to undertake AMHP training (if not already qualified)

Work Environment

Community-based role across Camden, including home visits, hospitals, and community settings. Hybrid working available (up to 2 days from home). Regular lone working required.
